Please download these forms which must be handed in at the time of your check-in at our events! EVERY TEAM MEMBER MUST FILL OUT A WAIVER FORM IN ORDER TO PARTICIPATE. Once a form has been filled out by your squad we will keep it on file! Only ONE WAIVER needed per year!

All coaches must fill out one Code of Ethics Form PER YEAR!
